"What do I need to do before my first appointment?"

 

In order to make the most of your time at Litchfield Family Practice Center, we encourage you to complete the following tasks prior to your visit:

 

• Verify and familiarize yourself with your insurance coverage. If you have any questions please contact our Insurance Department at (217) 324-1041.

 

• If you have medical records at another physician's office, ask them to copy or transfer those records to Litchfield Family Practice Center, ensuring we have a comprehensive medical history.

 

• Download the new patient forms listed on the forms page and fill them out prior to coming to your first appointment. You can either drop off this paperwork at LFPC ahead of time or come 15-20 minutes early for your appointment to submit the paperwork to our Check-In staff, so it can be entered into the system before you meet with the provider.
